Our mission is to transform the CHD experience by uniting
heart warriors, families, caregivers, foundations, organizations, researchers, and medical professionals into one powerful, connected community.
At the core of our mission is the Centralized Hub—a dynamic, collaborative space where heart warriors and families are at the center , ensuring that their voices, experiences, and needs shape the future of CHD care, research, and advocacy.
We are eliminating fragmentation by breaking down silos and fostering direct collaboration between those shaping the future of CHD care and those living it every day.
Through our Genome of Values and Principles of Adaptation, we ensure that every individual, organization, and professional has access to the resources, support, and advancements they need —all in one place.
We are committed to:
✔Building full transparency and access for heart warriors, families, and caregivers—ensuring they have a direct line to research developments, clinical trials, treatment advancements, advocacy efforts, and the organizations working to make a difference. By eliminating barriers to information, we empower every individual to take an active role in their CHD journey.
✔Placing heart warriors and families at the center of the CHD ecosystem—connecting them with researchers, medical professionals, and organizations to foster collaboration that ensures real-world needs and experiences drive advancements in care, treatment, and advocacy.
✔Providing unwavering support that grows with every stage of the CHD journey, ensuring that no one faces this experience alone.
✔Empowering action through advocacy, fundraising, and education, allowing individuals and organizations to
create tangible change together.
✔Accelerating research, improving medical care, and shaping policy by integrating real-life CHD experiences
into the larger conversation, ensuring that the needs of the CHD community directly influence medical progress.
✔Amplifying voices to ensure that no heart warrior, family, or caregiver is left unheard or unsupported.
By uniting the CHD community, empowering individuals and organizations, and driving systemic change, we are building a future of strength, innovation, and collective impact.
Together, we are not just changing lives—we are redefining the future of CHD.
Welcome to CHD Connects Hearts, a personal note from my heart to yours. I’m Momma C—mother, grandmother, and businesswoman, whose world shifted when my grandson, Hunter, was diagnosed with Hypoplastic Left Heart Syndrome. Our journey has been filled with both profound challenges and transformative joy, reshaping the fabric of our family's life.
My experience of helplessness during Hunter’s early days, accentuated by the isolation of COVID, underscored the urgent need for support networks for families facing congenital heart defects and disease. Witnessing Hunter’s courage through his Fontan procedure cemented my resolve to act.
CHD Connects Hearts emerged as a heartfelt mission to provide understanding, resources, and community for families like ours and a future place for my grandson, Hunter, to connect to others like him. It is a space designed for connection, offering lifelines of support, and nurturing a future where no family or individual journeys alone.
Together, we strive for compassion-driven change, with the hope of one day saying, "Change has come."
